Job Title: Nurse Case Manager – Complex Home Care
Location: Hybrid across Yorkshire, based in Huddersfield
Hours: Full-Time (35+ hours per week)
Salary: Up to £38,000 (plus bonus)

    The Role:
    As a Nurse Case Manager, you’ll take on a key leadership position within the team, ensuring the delivery of top-tier care to clients with complex needs. You’ll manage and oversee a caseload of service users, providing both clinical and holistic support, while working collaboratively with a range of professionals to ensure the highest standards of care.

    You’ll be entrusted with the responsibility for maintaining and setting high-quality care standards, monitoring clinical outcomes, and fostering a culture of continuous improvement. Your expertise will guide the development of personalised care plans, ensuring compliance with CQC regulations and best practice guidelines. Additionally, you’ll lead staff training and development, enhancing the capabilities of your team and ensuring they are equipped to provide safe and effective care.

     

    Key Responsibilities:

    • Oversee and manage a caseload of clients, ensuring comprehensive and tailored care for each individual
    • Lead and monitor the quality of care provided, ensuring it aligns with national standards and organisational goals
    • Develop and review care plans and risk assessments in collaboration with multi-disciplinary teams (MDT)
    • Manage, supervise, and support the clinical staff, ensuring their competencies are maintained and developed
    • Co-ordinate complex care package transitions and ensure continuity of care, liaising with families, MDTs, and clients
    • Provide training, supervision, and one-on-one support to non-qualified staff to promote skill development and excellence in care

    About You:
    We’re looking for a Registered Nurse (Band 6 or above) with a passion for high-quality, person-centred care. You’ll need a full UK driving license and access to your own vehicle, as the role involves travel within the region. Experience with tracheostomy and ventilation care is highly desirable but not essential.
