Multi-Trauma Case Manager – Remote – West Midlands

Location: Home-based with travel across the West Midlands 
Hours: 37.5 hrs/week
Salary: £39,000 - £45,000 (depending on experience)

 

Remit

Compass Associates is proud to be working with one of the UK's leading independent providers of rehabilitation and case management services, to recruit new Case Managers to join their multi-trauma team.

 

This is an exciting opportunity for experienced clinicians who are passionate about supporting individuals following serious injury and helping them achieve the best possible outcomes. You will support clients who have sustained complex and, in some cases, life-changing injuries, including orthopaedic trauma, internal injuries, musculoskeletal conditions and mild traumatic brain injuries.

 

Your role will involve undertaking Immediate Needs Assessments (INAs) and proactively managing rehabilitation programmes for clients within their homes and communities. You will work closely with clients, families, healthcare professionals and other key stakeholders, you will coordinate and deliver person-centred rehabilitation plans tailored to each individual's needs.

 

Location

This is a home-based role but you will be required to travel to visit clients within a 2 hour radius of your home. Candidates must  be based in the West Midlands/Shropshire such as Birmingham, Wolverhampton, Stoke-on-Trent, Stafford, Solihull, Wolverhampton, Dudley, Shrewsbury, Telford and surrounding towns.

Essential Criteria

  • Qualified Nurse, Occupational Therapist, Physiotherapist or Social Worker
  • Current HCPC, NMC or SWE registration
  • Minimum of two years' post-registration experience working with patients who have complex orthopaedic, multi-trauma, MSK and/or internal injuries
  • Full UK driving licence and own car

 

Interview Process

Two stage process:

  • 1st stage – informal Teams call with the Clinical Manager or a Clinical Lead
  • 2nd stage – formal Teams interview including a case study
